We did not buy tickets in advance , because it was a spur of the moment decision to visit, we just walked up and purchased out tickets when we arrive, there was no line at the time and we quickly got in, but when we left there was a line but it was not that long.
According to their website, yes, with a reservation. This is from their website: Access to the museum is free for all visitors on the 1st Sunday of every month. Reservation is compulsory.

No reservation required. Musée d'Orsay: Includes access to the Permanent collections and the Temporary exhibitions, subject to availability.
